Register as a New Patient

Providing you live within our catchment area and our Practice is accepting new patients you will be able to apply to join the Practice.

Please find the links to our registration forms which are listed below, print these and return to your desired branch, otherwise our reception staff can supply you with a copy. 

Please be aware that New Patients will not be fully registered until they have filled in the relevant forms.

Please note: We operate two sites, and although you have a usual branch, we cannot always guarantee that an appointment, at you usual branch, can be offered at the earliest convenience. We may offer you appointments at either location, however, if you prefer to only visit your usual branch, please be aware that this may cause a wait for your appointment.

Booking an Appointment

Please see below how our appointment request system works: 

 

  • The appointment request system will be open between 7:30am-10:00am daily. Clinicians will start triaging at 8am.
  • Once your request is received, urgent requests are selected as priority by the team and an appointment booked appropriately. You will be contacted on the same day to let you know when your appointment is.
  • Non-Urgent appointments will have an appropriate appointment booked with a regular or the same GP for continuing issues. You will be contacted on the same day to let you know when your appointment is.
  • If we reach our maximum number of triage requests before 10am, will we have to close off access early for that day.
  • Once we have closed off access, there is still the ability for a period after this where urgent considered matters can still be triaged by the GP. For this, contact the practice on 01246 244040 and our team will add your request onto the GP's urgent triage list.
  • There may be times when we have reached triaging capacity and all of our Urgent appointments have been filled. 
  • After this, our team will then signpost in the direction of other healthcare settings, such as NHS 111, Minor Injuries Units, Accident & Emergency or community pharmacists.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
